Master Form .PHP has a two-click CSV download from the MySQL table where the form data is stored. Master Form V4 can store form data directly to plain text files, CSV or other plain text formats.

Secure Form Software
Master Form PHP V3 is secure.
It has auto-submission protection. It has cross-site scripting protection (protection from XSS attacks). And it has other protection that can be selected in the Master Form PHP V3 control panel.
Spam Protection
Master Form PHP V3 has automatic form submission protection (without using an annoying visual CAPTCHA system).
Forms are submitted directly to the thank-you/confirmation page. Nowhere in the form or thank-you page is the actual location of the Master Form PHP V3 software revealed to spammers' robots.
IP addresses, email addresses, and words/phrases may be banned. Information from form submissions containing banned text is not recorded and no email is sent.
Versatile — Handles Most Forms
Chances are, all your forms can be handled with this Master Form PHP V3 software. Exceptions would be forms requiring special processing, such as quiz forms that generate graphs or subscription forms that submit to specialized emailing software.

MySQL
Information submitted with a form is stored in a MySQL database. MySQL tables and columns are created automatically.
The Master Form PHP V3 control panel has a link for downloading a CSV file of the form information stored in MySQL database tables. (CSV files can be imported into Excel, OpenOffice.org, and other popular spreadsheet software.)
Emails
One, none, several, or many emails can be sent with every form submission.
The emails are custom formatted, containing the language you want, the way you want it. The email may optionally include information submitted with the form.
Emails can contain results of calculations made on form fields. Example of use: Number of t-shirts ordered. Order subtotal. Amount of tax or VAT. Order grand total.
Emails may be HTML or plain text.
Emails may contain attachments. Attachments can be files uploaded with the form, copies of files on the server, and/or documents obtained from the Internet with an http://... URL.
Custom Thank-you Pages
Thank-you pages may be customized with any information submitted with the form. Results of calculations made on form fields can be printed.
